The breach of the triggers identified in these Guidelines should prompt the competent authorities (a) to further investigate the situation, if the cause of the breach is not yet known, and (b) taking into account the urgency of the situation and the magnitude of the breach within the overall situation of the institution, to make a decision on whether to apply early intervention measures.
